Terms & Conditions

Please read this information carefully and be aware in registering for participation in this program, you will be waiving and releasing all claims for injuries you might sustain arising out of your participation in the program you have registered for.

Program Purpose – Our program is designed to give students a safe place to go after school when they are unable to go home. Participants will experience organized activities, free play, homework time, homework assistance, crafts, games, snacks, and much more. All activities and planned and supervised by our trained staff.

Program Begin and End – The program begins immediately after school and ends at 6:00

p.m. on days when school is in session.

Program Location – The program will be held at the Peotone Elementary School. Students attending from Peotone Intermediate will be bussed to PES daily.

Tentative Schedule – This is a sample of how our schedule might look. We will incorporate a variety of activities to meet the needs of the students. Friday’s will include more fun time.

 

3:00 – 3:30 Wash up, get settled, short activity
3:30 – 3:45 snack time
3:45 – 4:15 wellness or structured activity
4:15 - 4:30 chill break
4:30 – 5:00 homework or quiet activity time
5:00 – 5:15 free time
5:15 – 5:45 activity time
5:45 – 6:00 Clean up & pack up

 

Early Dismissal Days – The program will be offered on early dismissal days.

Reminder - If there is no school, there is no Rec Care!

Program Payment – Payment is expected in advance of attendance. Failure to keep payments current will result in the removal of the child from the program.

Day Punch Card – Purchasing the 20-day Punch Card is the easiest way to pay for Rec Punch cards will be kept at the program site and staff members will “punch” the card every day your child attends. When your card is running low, with 5 punches left, staff will give you a reminder when you pick up your child.

Late Pick Up – Beginning at 6:00 p.m. a $1.00 per minute fee will be charged. Late fee payment must be made before the child may return to the program. Children who are picked up late more than four times may be asked to leave the program.

School Communication – The parent or guardian MUST let the school know your child will be participating in the Peotone Park District After school program. We are not responsible for children who are sent home due to a lack of communication with the school district.

Program Absence – You must call the Park District before 12:00 p.m. on regular school days to report a student's absence. If your child is scheduled to attend and does not, the regular program fee will be assessed and must be paid before the next day in attendance.

Notice of Attendance – If your child is not regularly attending the program and you would like for them to attend, attendance notice must be given by 11:00 a.m. to ensure availability.

Snacks – A daily nutritional snack will be provided daily excluding Fridays and other special

days where a “fun” snack will be provided.

Lunch – Lunch is not provided! Please send a lunch with your child when needed. Snow Days – If school is canceled before or during the school day, there will be no Rec Care.

Outside Play – Please have your child dress appropriately for the weather. We will spend time outside when possible (25 degrees and warmer). Make sure coats, hats, etc. are clearly marked with the child’s name for easy identification. Please provide a doctor’s note if a child has a medical condition that prohibits them from going outside.

Illness – The Rec Care staff has the right to refuse any child who is ill or has a fever. If a child becomes ill during the program, a parent will be notified and asked to pick up the child immediately. If parents are unavailable, emergency contacts will be notified for pick up. A child must be fever-free for at least 24 hours before returning to the program. In case of contagious disease, please contact Peotone Park District Manager immediately.

Personal Belongings – The Rec Care staff is not responsible for lost, stolen, or damaged toys, electronics, or other items brought from home. We suggest all personal items be left at home.

Cell Phones – The Rec Care staff is not responsible for lost, stolen, or damaged cell phones. Children will be encouraged to keep cell phones in their backpacks during Rec Care and will only be allowed to be used under staff supervision.

Behavior Management – The Rec Care program promotes positive behavior and good decision-making. Guidelines for behavior management are established to help create a system for dealing with inappropriate behavior during the program. Behaviors deemed inappropriate include but are not limited to offensive or abusive language, aggressiveness towards participants or staff, or other actions disruptive to program activities.

Staff is instructed to use the following corrective actions, depending upon the severity and consistency of the behavior:

  1. Verbal warning to stop the behavior
  2. Withdrawal of privileges or participation in Discussion of behavior.
  3. Complete removal for activity and parent
  4. Suspension or removal from the

In case of behavior that is severely disruptive or when participants or staff safety is an issue, parents will be contacted to assist with correcting the situation. Parent cooperation and correction of any inappropriate behavior is expected and appreciated. Prolonged misbehavior may result in a child’s dismissal from the program.

I recognize and acknowledge that there are certain risks of physical injury to participants in the above program and agree to assume the full risk of any injuries, damages, or loss regardless of severity that I may sustain as a result of participating in any and all activities connected with or associated with this program.

I agree to waive and relinquish all claims I may have as a result of participating in this program against the District and its officers, agents, servants, and employees. I do hereby fully release and discharge the District and its officers, agents, servants, and employees from any and all claims from injuries, damage, or loss which I may have or which may accrue to me and arising out of, connected with, or in any way associated with the activities of the program.

As a participant in a program or activity of the District, I hereby grant the District permission to use my or my image, video form, or voice in photographs, videotapes, Internet website, or other materials prepared or released by the District from time to time, for promotional, safety or instructional purposes.

I understand that such materials will be used and shown in whole or in part as the District sees fit. By this permission and release, I hereby release and discharge the District, its officers, employees, and agents from any and all claims or actions resulting from the use of such materials by the District.

In the event of any emergency, I authorize District officials to secure from any licensed hospital; physician, and/or medical personnel any treatment deemed necessary for my immediate care and agree that I will be responsible for payment of any and all medical services.
